10% discount when the year tuition is paid in full, 5% discount when the half year tuition is pain in full, 10% family, military, and for addt'l classes taken by same child.

• Any Accounts not paid by the 15th of each month will be charged a $15.00 late fee
• Students with account balances of >90 days will not be allowed to participate in class until due is paid
• All returned checks will be charged a $25.00 NSF fee

Breakin Beats Dance Studio may use the photographs of my child(ren) named above in its promotional materials and publicity efforts. I understand that the photograph(s) may be used in a pubilication, print ad, direct mail piece, electronic media or other form of promotion. I release Breakin Beats Dance Studio, the photographer, their offices, employees, agents, and designees from liability for any violation of any personal or proprietary right I may have in connection with such use. I will make no monetary or other claim against BBDS for the use of the photograph(s)/video.

I hereby authorize the staff and director, representing BBDS, to give consent for any and all necessary emergency medical care for my child(ren) while said child(ren) are in the custody of BBDS personnel. I also hold BBDS or any other BBDS personnel harmless in such an event.

We understand that situations may arise which prevent continued participation. Those who wish to terminate their enrollment should notify the office of BBDS in advance. Any tuition paid is non-refundable. In addition, my child's enrollment will be terminated by BBDS if my child misses 4 consecutive classes without prior notice to the office. I will be responsible for any and all tuition up to the date that my child's enrollment is terminated.
